How do I set up a JISC Online Survey?

Answer

In order to set up a JISC Online Surveys account, you will be required to complete the JISC Online Account Request form on Digital Services Portal page. Log in using your usual University details if prompted to do so.
 

The form asks you to indicate who the account is for (yourself, or the student if you are a staff member applying on a students' behalf), and list their University email for them to be given a JISC Online Surveys account.
