How do I set up a BOS/JISC online survey?

In order to set up a JISC Online Surveys account, you will be required to complete the 'JISC Online Account Request' form on Digital Services Portal page:
https://help.coventry.ac.uk/home
Here is a direct link for the "JISC Online Account Request " form.
Please then indicate who the account is for and list their University Email for them to be given a JISC Online Surveys account.
